TERMS OF SERVICE

By using our website, submitting a consultation form, or scheduling an appointment with Legacy Tattoo, you agree to the following Terms of Service. Please read them carefully.
1. Services
Legacy Tattoo provides professional tattoo and piercing services at 236 25th Street, Ogden, UT 84401. All services are performed by licensed and experienced artists and piercers.
2. Age Requirement
You must be 18 years of age or older to receive tattoo or piercing services at Legacy Tattoo without parental or legal guardian consent. Clients between the ages of 16 and 17 may receive certain piercing services with a parent or legal guardian present and providing written consent. We reserve the right to request valid government-issued photo identification at any time. Legacy Tattoo reserves the right to refuse service to anyone who cannot provide valid proof of age.
3. Consultations
Submitting a consultation form does not guarantee an appointment or a specific artist. Consultations are reviewed and responded to within 48 hours. Matching with an artist is subject to availability and artistic fit.
4. Deposits
A deposit is required to secure your tattoo appointment. Deposit amounts vary based on the scope and complexity of the tattoo. Your artist will communicate the deposit amount during the consultation process.
All deposits are non-refundable under any circumstances.
5. Cancellations and No-Shows
We understand that life happens, however Legacy Tattoo has a strict cancellation and no-show policy:
- Any cancellation, regardless of reason or timing, results in the automatic forfeiture of your deposit.
- No-shows result in the automatic forfeiture of your deposit.
- There are no exceptions to this policy and no refunds will be issued.
- A new deposit will be required to reschedule any future appointment.
6. Health and Safety
Clients are responsible for disclosing any medical conditions, allergies, or skin conditions that may affect the tattooing or piercing process. Legacy Tattoo follows all applicable health and safety guidelines. We reserve the right to refuse service to anyone whose health condition may pose a risk to themselves or our staff.
7. Right to Refuse Service
Legacy Tattoo reserves the right to refuse service to any individual at our sole discretion, including but not limited to individuals who are intoxicated, behave inappropriately, or request designs that violate our studio policies.
8. Liability Disclaimer
Tattooing and piercing are permanent body modifications. Legacy Tattoo is not liable for individual reactions to ink, jewelry, or aftercare products. Clients are responsible for following all aftercare instructions provided by their artist or piercer. Touch-ups resulting from client negligence or failure to follow aftercare instructions are not covered.
9. Intellectual Property
Custom tattoo designs created by Legacy Tattoo artists remain the intellectual property of the artist. We reserve the right to photograph completed work for portfolio and promotional purposes unless the client expressly requests otherwise in writing prior to the appointment.
10. SMS Communications
By providing your phone number, you consent to receive SMS communications from Legacy Tattoo related to your appointment and consultation. You may opt out at any time by replying STOP.
11. Changes to Terms
Legacy Tattoo reserves the right to update these Terms of Service at any time. Changes will be posted on our website with an updated effective date.
